Community Health Partnership - “CHP Serves Members.”

Project Duration: 6 Months

Community Health Partnership is part of the Wisconsin Partnership Program, whose goal is to provide services that allow seniors and the disabled to remain in their homes. CHP takes a team approach with physicians, nurses and other health care personnel as well as provides shopping and housekeeping assistance to program members. This video was used both as an introduction to new clients and employees as well as an informational piece to legislators.
